Health Ambassadors Connecting Homebound Older Adults to Primary Care
The In The Community (ITC) pilot program is here! Hosted by Gotcare and the Women’s College Hospital, ITC will connect primary care for homebound older adults living in the Mid-West Toronto area, aiming to support patients with complex needs. At no direct cost, patients may be referred to the program via a nurse navigator, a home care coordinator, their family doctor, or another member of a primary care team.

I’m a ClientI’m a Primary Care Provider
Eligible individuals include:
- People 65+ (or 55-64 with geriatric presentation) with chronic conditions (e.g. hypertension, CHF, COPD, asthma, diabetes, skin issues) and mobility deficits
- People who have difficulty following up on care, frequent ED visits, or risk for ongoing visits
- People capable of using simple in-home monitoring equipment as determined by their healthcare provider
- People with no or limited cognitive impairments who can provide informed consent
