The In The Community (ITC) pilot program is here! Hosted by Gotcare and the Women’s College Hospital, ITC will connect primary care for homebound older adults living in the Mid-West Toronto area, aiming to support patients with complex needs. At no direct cost, patients may be referred to the program via a nurse navigator, a home care coordinator, their family doctor, or another member of a primary care team.

Eligible individuals include:

• People 65+ (or 55-64 with geriatric presentation) with chronic conditions (e.g. hypertension, CHF, COPD, asthma, diabetes, skin issues) and mobility deficits
• People who have difficulty following up on care, frequent ED visits, or risk for ongoing visits
• People capable of using simple in-home monitoring equipment as determined by their healthcare provider
• People with no or limited cognitive impairments who can provide informed consent
